Koumajou Remilia: Scarlet Symphony is a 2D action-platformer that merges gothic aesthetics with bullet hell elements, offering a challenging experience reminiscent of classic Castlevania titles. Players assume the role of Reimu Hakurei, a shrine maiden investigating the reemergence of the Scarlet Mist in Gensokyo. Her journey leads her to the Scarlet Devil Mansion, where she confronts various inhabitants, including formidable bosses like Remilia Scarlet.

The gameplay is structured around eight linear stages, each culminating in intense boss battles that incorporate bullet hell mechanics, requiring precise movement and timing. Reimu is equipped with a purification wand for close combat and ofuda for ranged attacks.
