The title takes place before the events of Shadow of Chernobyl, in the very same zone that was featured in the first game. Players take control of a mercenary known as "Scorpion" who must scavenge supplies, explore dangerous environments, and combat hostile mutants and other survivors while struggling to survive within the contaminated Zone.

Players will find themselves being challenged not only by foes from outside but also from within as they are constantly being tested by rival factions and survivors. The game’s atmosphere is intense, with an attention to detail that creates a believable post-apocalyptic world for gamers to explore.
